All grantees must review and adhere to these crediting requirements for all Performing Arts Fund supported engagements. Includes links to required logos and examples of appropriate crediting.

Final Report
Submit an eGRANT Final Report and mail the signed Certification Page with your final report package to be received in Arts Midwest’s office within 30 days following the conclusion of your engagement. We will send you an e-mail reminder. Go to eGRANT.
